We understand that returning home or to another familiar environment often provides an immediate source of comfort and healing and we want you to benefit from this. You may be ready to leave Strong Memorial Hospital but still need additional health care services such as home health care or assisted living to ensure your safety. Throughout your hospital stay, your potential needs for these services will be assessed by your primary health care team and all of your questions and concerns will be addressed quickly and efficiently by the home care staff. At the time of discharge, your specific discharge needs will be assessed and all information will be shared with you and your primary support person(s).
