Mission:

The Mission of The South DeKalb Center for Healthy Living is to provide quality primary and preventative healthcare services to DeKalb County Residents and surrounding areas; to address the health and social service needs of this population and to provide health education to the community at large. 

Vision:

The Vision of The South DeKalb Center for Healthy Living is to create a community culture where all strive for good health and well being; where illness and disease is prevented or controlled; and healthy living is the way of life.

Goals:

 The Goals of The South DeKalb Center for Healthy Living are to: 

- Prevent illness by performing health screenings, providing health education and encouraging life style modifications that promote wellness;

 - Increase positive outcomes in patients with chronic diseases by providing regular screenings and aggressive case management;

 - Provide High quality comprehensive primary care and preventive health services to those who could not otherwise afford it.
