History and Mission:

Neighborhood Family Practice (NFP) has been serving Cleveland's near west side with quality primary health care services for over 30 years. Our mission is to partner with the community for everyone's best health. NFP began as a small storefront practice on Storer Avenue in 1980. The organization has grown substantially and now serves over 14,000 people per year. NFP became a federally qualified health center in 2000. NFP has continued to grow to meet the needs of its urban community and, in 2005, opened its second health center in the Tremont neighborhood. NFP opened its third health center in the Detroit Shoreway neighborhood in August 2012.

The target population within the eight statistical planning areas of the near west side is comprised of a very diverse mix of residents. The health center serves one of the largest Hispanic communities in Ohio, a population that continues to grow within the NFP service area. NFP also serves Cuyahoga County's refugee community.

NFP's staff and board maintain a strong focus on preventative care, treating chronic disease, mental health and substance abuse issues, and take a highly collaborative approach to solving health care and quality of life issues. NFP's midwifery services have continued to grow significantly with the implementation of the Centering Pregnancy Program and are addressing an ever-increasing need within the target population.

The health center received Joint Commission accreditation in April of 2008 and received an excellent review when re-accredited in 2011. This demonstrates NFP's emphasis on improving patient care and the efficiency and compliance of the overall delivery system. In November 2010, NFP converted to electronic medical records, taking its level of quality and efficiency to an unprecedented level. In 2012, as a part of its strategic goals, NFP achieved NCQA Patient Centered Medical Home Recognition because of its commitment to being a medical home to all patients. NFP was the first primary care practice in Ohio to achieve the recognition from NCQA at Level 3 - the highest level - under the most current 2011 standards.
