Health Centers Detroit Foundation, Inc. is a 501c3 organization based in the City of Detroit Michigan. We do business as Health Centers Detroit Medical Group (HCDMG) and operate as a Federally Qualified Health Center (FQHC) look-alike. We have been committed to, and serving the people of, the City of Detroit since 1998.

HCDMG, guided by strong leader ship team and a nationally recognized leader and CEO, Dr. Herbert Smitherman, Jr. M.D., M.P.H., has always been on the forefront of public health policy debate, pushing for the kind of quality health care which should be accessible to all.

As Federally Qualified Health Center look-alike we at Health Centers Detroit Medical Group have chosen to operate in areas that are Medically Undeserved Areas (MUA’s) and populations (MUP’s). We operate under guidelines which the Health Resources and Services Administration (HRSA) puts forth and as such have a Board that is composed of patients that use HCDMG services and meet on a regular basis to hear reports on the operation and well being of our organization and its continued commitment to serve the population of Detroit and surrounding areas.

Health Centers Detroit Medical Group has also started the certification, and is deeply involved in the process, of becoming a patient-centered medical home or PCMH. Under this initiative, started in June of 2011, emphasis has been placed on coordinated care where all aspects of a patients healthcare and medical issues spectrum are treated and coordinated under one roof. HCDMG has long been a proponent of this kind of medical care since the groups inception in 1998.
