Hazel Crest Community Health Center
The Hazel Crest Community Health Center – located in Suite 1150 of the southernmost building on the campus of Advocate South Suburban Hospital – opened in March 2007. The following year, the clinic added obstetrical and gynecological (OB/GYN) care to its menu of services.
