First Choice Primary Care

We’d like to be your medical home. We’ll be here for you when you are sick, and help you locate a specialist if that’s what you need. We can also help you learn more about how to stay healthy, and manage any chronic problems, like diabetes or high blood pressure.

What is a community health center? We are a private non-profit organization, controlled by a community board of directors. They make sure that the services are high quality yet affordable. Over half of our board members are patients here themselves.
