About Us:

Eastmont Wellness Center is part of Alameda Health System’s (AHS’s) network of community-based ambulatory clinics that expand access to primary care and AHS medical specialties.

Community-based care is provided to all residents of Alameda County by an interdisciplinary team of physicians and a variety of other healthcare professionals and specialists. We create a unique partnership with patients, providing an integrated service model that connects them with the care they need in a timely manner, including referrals for specialized care.
