Community Health Outreach

Community Health Outreach is the trade name under which WestJax Outreach, Inc., a Florida not-for-profit corporation and a registered 501(c)(3) tax exempt organization, operates and does business. With locations on the Westside of Jacksonville, and now in Putnam County, all services are provided free of charge and include food assistance, primary medical care, emergency dental treatment, confidential pregnancy testing and options counseling, baby clothes and other baby items.
