Community Health Center

At CHC, we provide an environment where patients feel comfortable and empowered to both manage their health and work towards a state of wellness. Through our partnership with medical faculty from Morehouse School of Medicine, we serve patients suffering from chronic diseases, such as diabetes and hypertension, as well as patients who show risks of developing these and conditions with an emphasis on prevention.
