What we offer...

 - Family practice medical office visits (no medical specialties are offered)

 - Prescriptions filled at CCCC's onsite pharmacy, as available

 - Assistance in obtaining more expensive medications through Prescription Assistance Programs

 - Limited lab work and medical testing

 - Dental extractions

 - Chiropractic visits, as available

 - Social workers and spiritual counselors, available upon request

 - Potential referral to specialists outside the clinic (Patient is responsible for all charges incurred with referrals

Who we serve... 

To qualify for services at CCCC, an individual must:

 - Be a Saline County resident

 - Have no health or dental insurance, including Medicaid or Medicare

 - Live in a household whose income is less than 200% of the federal poverty level
