As a National Committee for Quality Assurance (NCQA) Medical Home, we are here to serve you as your personal care team. We strive to exceed the standards around access and continuity, population and high-risk management, self-management and community care resources. All the while, we work to coordinate your care beyond our own system. Patient experience is extremely important to us, and we measure your satisfaction. Our on-site services include: primary and preventive care, pediatrics, outpatient mental health counseling, minor skin surgeries, obstetrics & gynecology and women's health services, and planned diabetic visits. Lab and x-ray services, and referrals to medical specialists, including dental care, can be coordinated to provide you high-quality, comprehensive care that meets your personal needs, as well as the needs of your extended care team.

Care will be provided to all, regardless of ability to pay. Financial assistance is available to those who are income-eligible and fees are charged in accordance with a sliding fee discount policy based on family size and income.
Charlestown Family Medicine is authorized to participate in the Anthem Pathways Network effective 1/1/14. Anyone who purchases insurance through the NH Insurance Marketplace may utilize any on-site Charlestown Family Medicine services at the Charlestown office. Any care required off-site will be coordinated through your medical home and primary care provider.
