Caswell Family Medical Center (CFMC) is a Community Health Center dedicated to providing quality health care to all segments of our community and the surrounding areas. We are proud of our accreditation by The Joint Commission. In October 2011, CFMC became one of the first ambulatory care centers in the United States to become designated by The Joint Commission as a Primary Care Medical Home.

Our mission is to serve our community with compassionate, customer-centered health care. Our dedicated providers and staff are committed to providing high quality and affordable health care while promoting health awareness and education. We offer a wide range of services to enhance the health of all individuals and families, including those with or without health insurance.
