Now Enrolling Participants
The Louisville Community of Care Project is now enrolling 100 participants who live in 40203 neighborhoods to walk on a health care journey. The goal is to reduce disparities in care for serious conditions such as high blood pressure, diabetes, and congestive heart failure.

The Louisville Community of Care Project is an initiative created by pastors in the 40203 zip code, medical providers and advocacy groups to address serious illness in the African American community. The primary objective is to increase access to high quality, person-centered care for African Americans experiencing serious illness by supporting faith-based organizations to connect members to care resources.

The Coalition to Transform Advance Care (C-TAC) and West Louisville pastors are coming together to provide you this opportunity at no cost.

Connect with the Louisville Community of Care Project
The Louisville Community of Care Project is a collaborative initiative by local pastors, medical providers, and advocacy groups in the 40203 zip code. The mission is to improve access to high-quality, person-centered care for African Americans facing serious illnesses. The project works with faith-based organizations to connect community members to essential care resources.

Program Supports
Participants will receive assistance from a Community Health Worker (CHW) who will help meet your care needs.
The project also offers financial support for eligible patients to cover screening and diagnostic tests, as funding permits.

How to Get Started

2. Request an appointment/service on the homepage.
3. Select the option for LCCP and complete the intake form.
