No matter where you are in your caregiving journey — starting to plan; taking care of a family member in your home, in a facility, or from a distance; or managing end-of-life caregiving responsibilities — having resources at your fingertips will make the process easier.

AARP developed this family caregiver guide with you, the caregiver, in mind and as a starting point to help you find the services and support you might need throughout your journey.

    Caregiver Support

    Resource Guide for NH Relative Caregivers
    Offers a range of local and state family caregiving resources, including family support groups, legal assistance, financial assistance, community action agencies, mental health care centers and educational resources.

    Coalition of family support and family resource centers and programs.

    ServiceLink New Hampshire (866-634-9412)
    Helps aging adults and persons with disabilities access long-term care services and Medicare and Medicaid information. The Caregivers Corner page offers specific caregiving resources.

    Finances

    Consumer Sourcebook – Schemes, Swindles & Scams (603-271-3641)
    Educates consumers on common types of fraud, including Ponzi schemes, pyramid schemes, phony job offers, false billings and more.

    Elder Abuse and Financial Exploitation Unit (603-271-3658)
    Investigates and prosecutes crimes involving elderly victims of abuse, neglect and financial exploitation. Provides contact details for the Bureau of Elderly & Adult Services and local county attorneys’ offices, where cases can be reported.

    New Hampshire Department of Revenue Administration (603-230-5920)
    Offers property-tax relief programs for qualified older adults.

    The Consumer Protection Bureau (888-468-4454 Consumer Hotline; 603-271-2643)
    Works to protect consumers from unfair or deceptive trade practices in New Hampshire by investigating and resolving complaints and educating consumers on their rights and about common scams.

    Legal

    New Hampshire Legal Aid (855-212-1234)
    Assists low-income adults who need legal representation, legal advice, information and support. Offers help with issues involving bankruptcy, housing, veterans, health care, employment and more.

    Offers legal assistance, legal advice, brief services and representation to those 60+ who qualify. Common areas of assistance include financial exploitation, food stamps, utility shutoff, disputes with long-term care facilities and more.

    Health

    ServiceLink New Hampshire (866-634-9412)
    Helps aging adults and persons with disabilities access Medicare and Medicaid information, long- term care services and caregiving resources.

    Housing and Care Facilities

    Housing and Urban Development (HUD) Housing Counseling (800-569-4287)
    Provides free or low-cost advice on buying a home, renting, credit issues, foreclosure and more.

    Housing Resources for Seniors (603-666-7510; 603-666-7518 (TTY)
    Provides and strengthens affordable quality housing for all New Hampshire residents. Website lists resources that are specifically helpful for older adults with housing needs.

    Offers non-medical personal care, including meals assistance, bathing, transportation, light housekeeping and more to homebound individuals or people with a disability.

    Granite State Independent Living (603-228-9680)
    Provides information, education, support, transition services and advocacy to aging adults and adults with disabilities who want to remain independent.

    NHCarePath (866-634-9412)
    Connects caregivers to services and resources for aging, disability, independent living, counseling, financial planning, home care, mental health, transportation and more.

    NH EASY Gateway to Services (800-852-3345)
    Helps older adults find services for health, financial, legal, housing, food, social and other resources.

    New Hampshire Department of Transportation: Public Transport (603-271-3734)
    Offers public transport options and provides information on regular services, plus specialized services for older adults and individuals with disabilities.

    New Hampshire Medicaid’s Non-Emergency Transportation Program (833-303-0653; 800-735-2964)
    Offers free rides to Medicaid recipients to and from a Medicaid- covered appointment through the One Call program or a managed care organization. Medicaid recipients who arrange their own transportation can also apply for reimbursements.

    NH Rideshare (800-462-8707)
    Offers ongoing and one-off transportation services to reduce transportation costs, traffic congestion and air pollution through shared rides.

    End of Life

    Home Health and Hospice Care (603-882-2941)
    Provides home health, hospice, palliative care and community services. Offers information for patients and their caregivers, including a caregiver training guide.
