The Chronic Disease Self-Management Program, originally developed at Stanford University, is a free educational program provided by Texas Health Resources in collaboration with Tarrant County Public Health, Tarrant County Area Agency on Aging, and Sixty & Better. The program includes six educational classes for adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease, chronic pain or diabetes. Each session teaches coping strategies for managing chronic conditions.
A few of the covered topics include:
-Techniques to deal with frustration, fatigue, pain and isolation
-Exercises for maintaining and improving strength, flexibility and endurance
-Appropriate use of medications
-Communicating effectively with family, friends and health professionals
-Weight management, healthy eating and nutrition
-Tips for evaluating new treatments
