Cancer Transitions Program-Moving Beyond Treatment: Next Steps Towards Survivorship

This psycho-educational program is designed to support, educate and empower people during the transition period following treatment— especially as people live longer and stronger lives after being treated for cancer. This program is ideal for patients who have recently completed treatment within the last six months.

Topics of discussion include:

-Exercise
-Nutrition
-Emotional and social issues
-Mindfulness

Participants will meet for a 1.5-hour sessions each week for six weeks. Expert presentations, group discussions, and an emphasis on setting and achieving active goals will ensure that participants receive a unique and enriching experience.
