Advance Care Planning: A gift for those you love

One of the best gifts family members can give to one another is the gift of advance care planning.  Taking the time to do this can help avoid family in-fighting and prevent families from having to guess what a loved one wants during a health crises or serious illness.

  • Learn how to document your wishes in an Advance Health Care Directive.
  • Get tips on how to engage in family conversations around this issue.  


The best way to help your loved one go through this process is to go through it yourself.  Participants in the session should review the conversation starter kit and Advance Care directive prior to the webinar. We recommend trying to complete the Advance Care Directive for yourself.  You will have the opportunity during the webinar to ask questions to help you complete it.

About the Speaker:

Hope Young is the Advance Care Coordinator for Kōkua Mau.  She provides community awareness and coordinates the Let’s Talk Story Program for the former speakers bureau. Hope joined Kōkua Mau in 2017 under a generous grant from the Stupski Foundation. Hope received her degree in Public Administration from the University of West Oahu, with a concentration in Health Care Administration. She has worked in various capacities with seniors and the caregiving community for over 18 years. Hope enjoys being out in the community and sharing the importance of Advance Care Planning and hopes to someday normalize “The Conversation” to ensure that everyone’s end-of-life wishes are expressed and honored.
